Please familiarize yourself with the following.

                    Your rights include:

  • Being treated with courtesy, dignity, and respect by member services
  • Access to a list of Community Care Associate providers
  • Choosing a Primary Care Physician (PCP) from our network
  • The option of changing your Primary Physician  
  • Participating with your doctor regarding your health care
  • Maintaining the privacy of your Protected Health Information subject to HIPPA regulations/guidelines
  • Access to HealthChoice Plan regulations & guidelines
  • Access to information about how to use CCA’s services
  • CCA's Compliance with HealthChoice Plan regulations


              Your responsibilities include:


  • Notifying your employer of address/telephone changes
  • Presenting your I.D. card to participating providers before treatment
  • Immediately reporting loss/theft of your Identification Card
  • Following HealthChoice Plan rules and regulations
  • Following CCA’s rules for receiving health care services
  • Assuring that no one else uses your card
  • Choosing a Primary Care Physician (PCP)
  • Treating Community Care Associates staff members with respect
  • Knowing what to do if Community Care Associates offices are closed
  • Accurately informing your PCP of your entire medical history
  • Completing required forms for treatment
  • Keeping your doctor’s appointments
  • Reporting any suspected fraud
  • Obtaining necessary pre-authorizations
