As a member of Community Care Associates, Inc. you have certain health care rights and responsibilities.
Please familiarize yourself with the following.
Your rights include:
The expectation of privacy
Being treated with courtesy, dignity, and respect
Choosing a Primary Care Physician (PCP)
The option of changing your Primary Physician (yearly)
Access to a list of Community Care Associate providers
Access to information about how to use CCAI’s services
Participating with your doctor regarding your health care
Maintaining the privacy of your Protected Health Information
Courteous service from the Customer Service Department
Compliance with Federal Law regarding hospital stays after delivery
Your responsibilities include:
Notifying your employer of address/telephone changes
Presenting your I.D. card to participating providers before treatment
Immediately reporting loss/theft of your Identification Card
Following Wayne County HealthChoice rules and regulations
Following CCAI’s rules for receiving health care services
Assuring that no one else uses your card
Choosing a Primary Care Physician (PCP)
Treating Community Care Associates staff members with respect
Knowing what to do if Community Care Associates offices are closed
Accurately informing your PCP of your entire medical history
Completing required forms for treatment
Keeping your doctor’s appointments
Reporting any suspected fraud
Obtaining necessary pre-authorizations
Contacting CCAI about changes in the following information: o Address o Phone Number o Eligibility
