MEMBER RIGHTS AND RESPONSIBILITIES

As a member of Community Care Associates, Inc. you have certain health care rights and responsibilities.

Please familiarize yourself with the following.

Your rights include:

 The expectation of privacy

 Being treated with courtesy, dignity, and respect

 Choosing a Primary Care Physician (PCP)

 The option of changing your Primary Physician (yearly)

 Access to a list of Community Care Associate providers

 Access to information about how to use CCAI’s services

 Participating with your doctor regarding your health care

 Maintaining the privacy of your Protected Health Information

 Courteous service from the Customer Service Department

 Compliance with Federal Law regarding hospital stays after delivery

Your responsibilities include:

 Notifying your employer of address/telephone changes

 Presenting your I.D. card to participating providers before treatment

 Immediately reporting loss/theft of your Identification Card

 Following Wayne County HealthChoice rules and regulations

 Following CCAI’s rules for receiving health care services

 Assuring that no one else uses your card

 Choosing a Primary Care Physician (PCP)

 Treating Community Care Associates staff members with respect

 Knowing what to do if Community Care Associates offices are closed

 Accurately informing your PCP of your entire medical history

 Completing required forms for treatment

 Keeping your doctor’s appointments

 Reporting any suspected fraud

 Obtaining necessary pre-authorizations

 Contacting CCAI about changes in the following information: o Address o Phone Number o Eligibility
